MINISTER ORDINATION QUESTIONNAIRE

The requirements for minister ordination or license in the CAC are to affirm the Apostles' Creed and the CAC Statement of Principles. The text of the Apostles' Creed can be found online. Here is one such text:

I believe in God, the Father Almighty,
Maker of heaven and earth.
And in Jesus Christ, His only Son, our Lord,
who was conceived by the Holy Spirit,
born of the virgin Mary,
suffered under Pontius Pilate,
was crucified, died and was buried.
He descended into hell.
On the third day He rose again from the dead.
He ascended into heaven
and sits at the right hand of God the Father Almighty.
From thence He will come to judge the living and the dead.
I believe in the Holy Spirit,
the holy Christian Church,
the communion of saints,
the forgiveness of sins,
the resurrection of the body,
and the life everlasting. Amen.

You will be asked to give the names and contact information of five people who will affirm your ministry calling. Also, please provide a description of the area(s) of ministry to which you feel called.

To complete this minister ordination form, please read the following affirmations and promises. If you agree, please print your name in and sign the statement below the affirmation, after you print out the ordination form..

"I affirm that the information contained in this minister ordination questionnaire is true. I promise to conduct myself in an ethical manner and live according to the precepts of the Christian faith. I affirm the Apostles' Creed and the CAC Statement of Principles."

Please send your completed minister ordination form and any attachments to me at the above email address.

Upon completion of these steps, and approval for ordination or licensing, you will be given a packet which will contain the following: a Certificate of Ordination or Certificate of License, a Letter of Good Standing, a Pastor Authorization Letter, and a notarized document setting forth the legal establishment of the CAC. I am requesting a donation for the Ordination/Licensing Credentials packet. For further explanation, please see the "Donations" page.
