MINISTER ORDINATION QUESTIONNAIRE

The only requirements for minister ordination or license in the CAC are to affirm the Apostles' Creed and the CAC Statement of Principles. The text of the Apostles' Creed can be found online. Here is one such text:

I believe in God, the Father Almighty,
Maker of heaven and earth.
And in Jesus Christ, His only Son, our Lord,
who was conceived by the Holy Spirit,
born of the virgin Mary,
suffered under Pontius Pilate,
was crucified, died and was buried.
He descended into hell.
On the third day He rose again from the dead.
He ascended into heaven
and sits at the right hand of God the Father Almighty.
From thence He will come to judge the living and the dead.
I believe in the Holy Spirit,
the holy Christian Church,
the communion of saints,
the forgiveness of sins,
the resurrection of the body,
and the life everlasting. Amen.


To complete this minister ordination form, please read the following affirmations and promises. If you agree, please print your name in and sign the statement below the affirmation, after you print out the ordination form..

Upon completion of these steps, you will be given a packet which will contain the following: a Certificate of Ordination or Certificate of License, a Letter of Good Standing, and a notarized document setting forth the legal establishment of the CAC.
